At Kings Support Group Ltd we consider young adults for supported housing if you are 18 years or over with mental health conditions, a physical or learning disability and you meet the following criteria: 

• You have a local connection to one of our covered areas (Please see the areas we cover in the About Us page.

• You’re in need of housing

• You have support needs

• Your needs are compatible with the services we offer (Please see our Resident Handbook in the Policies and Procedures page).

When making a self-referral, you must ensure you are well enough to manage independently in supported housing (except in the case of a physical or learning disability).

In the Upload section of of this self-referral form, please upload a copy of your care plan, risk assessment and any other relevant documents that can help your application. All the information provided will be considered in line with our eligibility criteria, as stated in our Residents Handbook.
